Subacute & Transitional Care Center is a 106-bed facility providing
inpatient care for patients with chronic and subacute conditions —
such as coma, ventilator dependence, or complex illnesses — that
require 24-hour medical supervision. With a focus on comfort,
recovery and rehabilitation, CSTCC blends the effectiveness of
acute treatment with the supportive environment of long-term care,
helping patients progress toward improved function and quality of
life. Perform physical therapy examinations, evaluations, and
individualized treatment planning to restore mobility and improve
physical function. Participate in interdisciplinary case
conferences, discharge planning, and the coordination of care
across the rehab team. Implement a plan of care for patients and
evaluate the effectiveness of it in an ongoing, systematic process.
Supervise physical therapist assistants, rehab therapy aides, and
student clinical experiences to ensure quality care and
professional development. Guide therapy and intervention through
evidence-based research As part of the Valley’s largest healthcare

meaningful recovery goals. Qualifications Education & Experience
Bachelor's Degree in Physical Therapy required 2 years of
experience required Licenses and Certifications PT - State of
California Physical Therapist License required BLS - Current Basic
Life Support (BLS) for Healthcare Providers by American Heart
Association (AHA) required
