As a Home Health Registered Nurse, here’s a big-picture view of
what you’ll do: Provide one-on-one, in house care for an adult
patient, including tracheostomy (trach) and gastrostomy tube
(G-tube) management, and medication administration Monitor vital
signs, assess patient conditions, and document findings accurately
in patient records Maintain open communication with the RN
supervisor, care team, and family members to ensure continuity of
care Carry out the patient’s plan of care, including therapeutic
interventions and rehabilitation exercises as prescribed Educate
the patient and family on the care plan, medication administration,
and home safety measures Follow healthcare regulations, infection
control protocols, and ensure proper maintenance of medical
equipment A few must-haves for Home Health Registered Nurses:
Graduate of an accredited nursing program and active RN license in
CA Minimum of 1 year of nursing experience, ideally in home
healthcare, skilled nursing, or an acute care setting CPR
certification Knowledge of state and federal home health
regulations Clinical proficiency in nursing specialty, strong
